![]() CSRs must be renewed at the same time as a physician's medical license (i.e., by October 31 of odd-numbered years) and can be renewed simultaneously online.In order to prescribe, administer or dispense controlled substances in Indiana, physicians must obtain a CSR from the Indiana Board of Pharmacy.Physicians with an inactive license may not maintain and office or practice charge a fee for a service that constitutes the practice of medicine or hold either a federal Drug Enforcement Administration Registration or an Indiana Controlled Substance Registration. The fee for an inactive license is half of the active license fee (i.e., $100). Physicians may choose to reduce their license status to inactive.Renewal fees are $200 (late fees are an additional $50).Physician licenses must be renewed by October 31 of odd-numbered years (i.e., every two years) and can be renewed online in most instances.The MLB can be contacted at (317) 234-2060 or visit the MLB website. Electrical Masters, Master Mechanical, Sheet Metal, Ventilation and Refrigeration exams must receive a score of 75% or higher on each part to pass the exam.You will receive immediate notification of your score at the end of the exam.Exam fee is non-refundable if you do not call within 24 hours of exam date to cancel or reschedule.You are not allowed to write information in the books or to add anything to the books with the exception of permanent tabs.You are allow to underline, highlight, and/or tab references prior to the exam.Study guide link is found at end of each license description.Once this is done, you will be contacted (via e-mail or phone) with approval to contact Ivy Tech at (812) 429-9852 to schedule and pay required fee for each test you are taking.
